By signing this form, I understand I am requesting cancellation of my website and services with 360 Media Co. I understand if I have other services with 360 Media Co., this does not cancel those services. I understand that once my website is cancelled, my website design and contents will no longer be available to me. I understand per my contract, I am to give a 30 day notice. I understand there are no refunds.
